Treatment with Apocynin or Picrorhiza kurroa decreases clinical and pathological features of parkinsonism

Abstract

This study evaluates the therapeutic efficacy of the NADPH oxidase inhibitor apocynin, isolated as principal bioactive component from the medicinal plant Picrorhiza kurroa, in a marmoset MPTP model of Parkinson’s disease (PD). The methoxy-substituted catechol apocynin has a similar structure as homovanillic acid (HVA), a metabolite of dopamine (DA). Apocynin acquires its selective inhibitory capacity of the reactive oxygen species generating NADPH oxidase via metabolic activation by myeloperoxidase (MPO). As MPO is upregulated in activated brain microglia cells of PD patients and in MPTP animal models, the conditions for metabolic activation of apocynin and inhibition of microglia NADPH oxidase are in place. Marmoset monkeys received oral apocynin (100 mg/kg; p.o.) (n = 5) or Gum Arabica (controls; n = 5) three times daily until the end of the study, starting 1 week before PD induction with MPTP (1 mg/kg s.c. for 8 days). Parkinsonian symptoms, motor function, home-cage activity and body weight were monitored to assess the disease development and severity. Post-mortem numbers of the tyrosine hydroxylase expressing DA neurons in the substantia nigra were counted. During the MPTP injections, apocynin limited the body weight loss and relieved parkinsonian symptoms compared to controls (Linear regression, P < 0.05) indicating a reduction of disease progression. During the last test week, apocynin also improved the hand-eye coordination performance compared with vehicle treatment (resp. 39.3 ± 4.5 % and 17.7 ± 6.7 %; P = 0.048) and improved the home cage activity with 32 % (P = 0.029), indicating anti-Parkinson efficacy. Apocynin also increased the number of surviving DA neurons in MPTP-treated marmosets with 8.5 % (P = 0.059), indicating a tendency towards a neuroprotective efficacy. In conclusion, compensation for the loss of DA and its metabolite HVA by apocynin mitigates the PD progression and limits the parkinsonian signs and motor-function deterioration.

Oxidative stress as a cause of nigral cell death in Parkinson’s disease and incidental lewy body disease

Abstract

We examine the evidence for free radical involvement and oxidative stress in the pathological process underlying Parkinson’s disease, from postmortem brain tissue. The concept of free radical involvement is supported by enhanced basal lipid peroxidation in substantia nigra in patients with Parkinson’s disease, demonstrated by increased levels of malondialdehyde and lipid hydroperoxides. The activity of many of the protective mechanisms against oxidative stress does not seem to be significantly altered in the nigra in Parkinson’s disease. Thus, activities of catalase and glutathione peroxidase are more or less unchanged, as are concentrations of vitamin C and vitamin E. The activity of mitochondrial superoxide dismutase and the levels of the antioxidant ion zinc are, however, increased, which may reflect oxidative stress in substantia nigra. Levels of reduced glutathione are decreased in nigra in Parkinson’s disease; this decrease does not occur in other brain areas or in other neurodegenerative illnesses affecting this brain region (i.e., multiple system atrophy, progressive supranuclear palsy). Altered glutathione metabolism may prevent inactivation of hydrogen peroxide and enhance formation of toxic hydroxyl radicals. In brain material from patients with incidental Lewy body disease (presymptomatic Parkinson’s disease), there is no evidence for alterations in iron metabolism and no significant change in mitochondrial complex I function. The levels of reduced glutathione in substantia nigra, however, are reduced to the same extent as in advanced Parkinson’s disease. These data suggest that changes in glutathione function are an early component of the pathological process of Parkinson’s disease. The data presented suggest (1) there is oxidative stress in the substantia nigra at the time of death in advanced Parkinson’s disease that manifests in terms of increased lipid peroxidation, superoxide dismutase activity, and zinc levels; (2) there is a major impairment of the glutathione pathway in Parkinson’s disease; and (3) alterations in reduced glutathione levels may occur very early in the illness.

AGEING AND PARKINSON’S DISEASE: SUBSTANTIA NIGRA REGIONAL SELECTIVITY

The micro-architecture of the substantia nigra was studied in control cases of varying age and patients with parkinsonism. A single 7μ section stained with haematoxylin and eosin was examined at a specific level within the caudal nigra using strict criteria. The pars compacta was divided into a ventral and a dorsaltier, and each tier was further subdivided into 3 regions. In 36 control cases there was a linear fallout of pigmented neurons with advancing age in the pars compacta of the caudal substantia nigra at a rate of 4.7% per decade. Regionally, the lateral ventral tier was relatively spared (2.1% loss per decade) compared with the medial ventral tier (5.4%) and the dorsal tier (6.9%). In 20 Parkinson’s disease (PD) cases of varying disease duration there was an exponential loss of pigmented neurons with a 45% loss in the first decade. Regionally, the pattern was opposite to ageing. Loss was greatest in the lateral ventral tier (average loss 91%) followed by the medial ventral tier (71%) and the dorsal tier (56%). The presymptomatic phase of PD from the onset of neuronal loss was estimated to be about 5 yrs. This phase is represented by incidental Lewy body cases: individuals who die without clinical signs of PD or dementia, but who are found to have Lewy bodies at post-mortem. In 7 cases cell loss was confined to the lateral ventral tier (average loss 52%) congruent with the lateral ventral selectivity of symptomatic PD. It was calculated that at the onset of symptoms there was a 68% cell loss in the lateral ventral tier and a 48% loss in the caudal nigra as a whole. The regional selectivity of PD is relatively specific. In 15 cases of striatonigral degeneration the distribution of cell loss was similar, but the loss in the dorsal tier was greater than PD by 21%. In 14 cases of Steele-Richardson- Olszewski syndrome (SRO) there was no predilection for the lateral ventral tier, but a tendency to involve the medial nigra and spare the lateral. These findings suggest that agerelated of pigmented nigral cells is not an important factor in the pathogenesis of PD.

Instructions

Ayurvedic medicine – This has been practiced in India for 5000 years. Parkinson’s symptoms are mentioned in ancient text under the name Kampavata. One such herb, Mucuna Puriens, is gaining attention in conventional circles as its effects mimic synthetic Levodopa, with fewer side effects.

Broad beans– Australian researchers discovered that broad beans are also an extremely effective natural source of L-dopa. The highest concentration of L-dopa is found in the pod so they are most effective when consumed whole.

St John’s Wort– Dopamine influences positive feelings in the brain, and since dopamine levels are low in Parkinson’s patients, depression is often a symptom. St John’s Wort is a herb that has been used in Europe for many years. It has been proven to be effective in alleviating depression and insomnia.

Botulinum toxin A– This is a bacterium that causes food poisoning (botulism) but has proven to be effective in reducing hand, head and voice tremors when in a weak solution.

Coenzyme Q10 (CoQ10) – This has been shown to have an effect on the symptoms of Parkinson’s Disease, however it is unclear whether it actually slows the disease or simply temporarily alleviates symptoms. The drawback is the massive dose required. The effective dose is approximately 1,200 milligrams a day, well above the 60 to 90 milligrams recommended by many alternative therapy advocates.

Accupuncture– Used for centuries in China to correct energy disturbances in the body. It has become a popular method of treatment for Parkinson’s sufferers the world over. So far there are no placebo controlled studies that show acupuncture can treat the motor control symptoms of the disease, but there is some evidence that it can assist with sleep disturbances. There is much anecdotal evidence to suggest that it may be effective in increasing feelings of well being and relaxation.

Massage– While not treating the symptoms directly, it can help reduce some of the discomfort associated with muscle stiffness that is commonly experienced by patients.

Alternative treatments for many diseases come and go. Some become fashionable for a short while only to be discredited and discarded. Others accumulate a growing body of scientific and popular support. It is to a large extent simply a matter of trial and error. Most alternative treatments are harmless, but some herbal remedies may interfere with medication, so be sure to consult your health practitioner for advice.

Lawsuit: Disclose PCB Levels in Fish Oil

A lawsuit brought by environmentalists in California claims popular brands of fish oil dietary supplements contain unsafe and illegal levels of the carcinogenic chemicals known as polychlorinated biphenyls, or PCBs.The lawsuit, filed in California Superior Court in San Francisco, targets eight supplement manufacturers or distributors – CVS Pharmacy, Rite Aid, General Nutrition Corp., Solgar, Twinlab, Now Health, Omega Protein and Pharmavite – for alleged violations of California’s Proposition 65, which requires that consumers be warned about chemical exposures.The plaintiffs are two citizen environmentalists and the Mateel Environmental Justice Foundation. Their initial testing found that levels of PCBs in supplements in popular fish oil products varied wildly, from about 12 nanograms per recommended dose in the best performer to more than 850 nanograms in the worst performer – a factor of 70.

But the suit claims that all of the manufacturers are in violation of Proposition 65 for not disclosing any non-zero PCB levels in their products.

The plaintiffs and their attorneys claim that that labels saying “Screened for PCBs” or “Treated to Remove PCBs” are especially problematic, because those labels imply – falsely – that PCBs have been removed entirely. They also believe that the manufacturers already have extensive data on the amount of PCBs present in their product.

Even the companies boasting the lowest contaminant levels have no incentive to release their data unless their competitors are forced to do so as well.

PCBs were officially listed as known carcinogens and known reproductive toxins in California two decades ago, making them subject to the California’s warning requirement.

Still, a representative from the Center for Science in the Public Interest, speaking to CBS News, pointed out two 2008 reports that found no unsafe levels of dioxin, mercury or PCBs in a wide array of products – one from Consumer Reports and another from the well-regarded supplement testing publication ConsumerLab.
